Transition-Age

Humana Community Services currently offers out-of-home, transition-age, mental health programs for youth aged 16-21 years. These intermediary models help youth transition from children’s services to adult services in London-Middlesex.

The programs provide a semi-independent living environment that is safe, stable, trauma-informed and meets the basic needs of the young person (such as food, clothing, safe shelter and medical care). 

This stepping stone to independent living provides youth with opportunities to practice life-skills and coping strategies with staff support in an environment that provides structure and routine that is age-appropriate.  Youth receive individualized program planning, support in managing mental health issues, and assistance in making a successful transition to needed services in the adult mental health system.

Program Funding

Transition-age programs are funded through the Ministry of Children, Community and Social Services (MCCSS). Passport funding is also provided to individuals to assist with recreation and leisure activities of interest to them.

  • Supports male identifying youth ages 16-18

  • Supports female identifying youth ages 16-18
